基于IMB 模型的过渡期护理方案在冠状动脉旁路移植术后合并2 型糖尿病ICU 患者中的应用

摘要：目的 分析冠状动脉旁路移植术（CABG）术后合并2型糖尿病（T2DM）的ICU患者应用基于信息-动机-行为技巧（IMB）模型的过渡期护理方案的作用。方法 选取2025.1~2025.6至我院就诊的45 例CABG 术后合并T2DM 的ICU 患者作为基础组，开展常规护理。选取2025.7~2025.12 至我院就诊的45 例CABG 术后合并T2DM 的ICU 患者作为研究组，开展基于IMB 模型的过渡期护理方案。组间对比自我管理能力、血糖控制情况、住院相关指标。结果 研究组自我管理能力评分均高于基础组（P<0.05 ）；研究组血糖指标水平均低于基础组（P<0.05 ）；研究组住院相关指标水平均低于基础组（P<0.05）。结论 CABG术后合并T2DM的ICU患者应用基于IMB模型的过渡期护理方案能够提高自我管理能力，降低血糖水平，改善住院相关指标，具有临床借鉴价值。

Application of a Transitional Care Program Based on the IMB Model in ICU Patients with Type 2 Diabetes Mellitus Following Coronary Artery Bypass Grafting

Abstract: Objective To analyze the effect of applying a transitional care program based on the Information-Motivation-Behavioral Skills (IMB) model in ICU patients with type 2 diabetes mellitus (T2DM) following coronary artery bypass grafting (CABG).Methods A total of 45 ICU patients with T2DM following CABG who were admitted to our hospital from January 2025 to June2025 were selected as the basic group and received routine nursing care. Another 45 ICU patients with T2DM following CABG who were admitted to our hospital from July 2025 to December 2025 were selected as the study group and received a transitional care program based on the IMB model. The two groups were compared in terms of self-management ability, glycemic control, and hospitalization-related indicators. Results The self-management ability scores in the study group were higher than those in the basic group(P<0.05 ); the glycemic indicator levels in the study group were lower than those in the basic group (P<0.05 ); and the hospitalization-related indicator levels in the study group were lower than those in the basic group (P<0.05 ). Conclusion The application of a transitional care program based on the IMB model in ICU patients with T2DM following CABG can improve self-management ability,reduce blood glucose levels, and improve hospitalization-related indicators, demonstrating value for clinical reference.
