专科小组全程护理管理在糖尿病患者自我管理中的应用 |

摘要：目的 探讨专科小组全程护理管理模式在糖尿病患者自我管理中的应用及效果。方法 选取我院2024 年7 月-2024 年12 月所收治糖尿病患者90 例，随机分为实验组和对照组，每组各45 例，对照组予以常规专科护理管理；实验组加入糖尿病专科小组全程管理模式，予以针对性专科管理内容，比较组间差异。结果 实验组的合理饮食、遵医用药、日常生活能力、社会心理、生理活动均显著高于对照组，差异具有统计学意义（P＜0.01）。实验组的空腹血糖、餐后2h 血糖均低于对照组，差异具有统计学意义（P＜0.05）。实验组的总体满意度高于对照组，差异具有统计学意义（P＜0.05）。结论 糖尿病专科小组全程护理管理模式有利于改善糖尿病患者自我管理行为，提升治疗依从性，可更好控制患者血糖水平。

Application of Specialist Team's Whole-Process Nursing Management in the Self-Management of Diabetic Patients |

Abstract: Objective To explore the application and effectiveness of a specialized team-based comprehensive nursing management model in the self-management of diabetic patients. Methods A total of 90 diabetic patients admitted to our hospital from July 2024 to December 2024 were selected and randomly divided into an experimental group and a ontrol group, with 45 cases in each group. The control group received conventional specialized nursing management, while the experimental group received a comprehensive management model led by a diabetes specialized team, with targeted specialized management interventions. Differences between the two groups were compared. Results The experimental group showed significantly higher scores in rational diet, medication adherence, daily living ability, psychosocial functioning, and physical activity compared to the control group, with statistically significant differences (P<0.01). The experimental group also had lower fasting blood glucose and 2-hour postprandial blood glucose levels than the control group, with statistically significant differences (P<0.05). Overall satisfaction in the experimental group was higher than that in the control group, and the difference was statistically significant (P<0.05). Conclusion The specialized team-based comprehensive nursing management model is beneficial for improving self-management behaviors and treatment adherence in diabetic patients, leading to better glycemic control.
